With you all the way, created by Novo Nordisk, is designed to help healthcare professionals provide children and young people with the necessary knowledge, skills and confidence to successfully manage type 1 diabetes at every stage of life.

The UK has the fifth largest population of children and young people with type 1 diabetes in the world, with 31,000 children with the condition in the UK.

The prevalence of diabetes in children and young people is steadily increasing. Supporting young patients and their families has never been more important.

A source of support

Created alongside a team of UK-based diabetes experts, the With you all the way resources have been developed in-line with recent changes in the clinical management of diabetes and the 2015 NICE guidelines.

The resource kit contains a range of age-specific booklets tailored to individual levels of development and understanding.  It is designed to facilitate patient-centred learning and enable children and young people to gradually take charge of their diabetes over time.

Goals of diabetes education

Goals of diabetes education is a structured education programme, designed to provide an overview of the key facts that your patients should understand as they journey from diagnosis through to transitioning from paediatrics to adult diabetes services.

To enable healthcare professionals to set age-specific goals, the materials are divided into 6 age groups (as listed in the key above).

The Goals of diabetes education materials provided for each age group are comprised of:
 

Guides for healthcare professionals

An overview of the key facts that your patients should understand in relation to their diabetes.  Use this guide to inform your own clinical practice and as part of a narrative discussion with your patients.
 

Handouts for parents and young people

Corresponding handouts to be provided to children and their families, to help them feel supported and allow them to gradually take charge of their own diabetes management.
 

Record sheets

A record sheet to log the level of knowledge and skill of your patients, and to set educational goals. 

Handouts can be personalised and used to help engage in a collaborative dialogue with your patients and their families.  This will help them to grow in confidence and understand the importance of self-care as a means to better health outcomes.
